Please bring the following items with you to your appointment:

• Patient registration forms if you haven’t submitted them prior to your visit.
• Dental Insurance Card (if applicable)
• Identification such as Driver’s License, Military ID or State ID

Hayes Family Dentistry is a private practice dental office who strives to care for every patient in a unique manner. All private insurance policies are welcome; however, we are in network with:

• Delta Dental Premier
• CIGNA PPO

Private pay is accepted and welcomed. We do not currently accept Medicare and Medicaid. Please call our office concerning and questions regarding your insurance plan.

The American Dental Association (ADA) guidelines recommend visits to a dentist at least twice a year for a check-up and professional cleaning. Our office also recommends a minimum of two visits per year.

Please call or text our office as soon as you determine that you have a dental emergency. We will be glad to work you in to our schedule if you have a dental emergency during regular business hours. After hours, please call our office for the doctor’s emergency contact number.
